What: Program for children entering foster care and children in families receiving Child Protective Services treatment.

Purpose: Optimize development for a group of at risk children by creating coordinated screening and treatment program for children entering foster care or Child Protective Services.
How:
- Assess the health and developmental status of children entering foster care and Child Protective Services.
- Create a plan of care to address special health needs, behavioral/developmental concerns, and referral to care.
- Establish a medical home for children.
Outcome:
- Increased identification of children with developmental and behavioral problems.
- Identification of a medical home for all children in foster care and Child Protective Services.
- Individual plan of care for each child.
- Secured referrals for needs that are indicated by screenings for developmental delays or emotional and behavioral conditions.
Cost/Term: $520,893 for a three-year period. October 2003-June 2007
